One shared picture of a care journey.
Medicare holds goals, medication plans, symptoms, appointments and notes in one place, so the people involved in a care journey work from the same information instead of trading versions of it.
Sign in the way you use Medicare
Each account is separate. Pick the one that matches how you are involved in the care journey.
Patients and families
Keep your own plan current, and support someone else when they need it. Shared sections keep relatives and professionals involved without anyone having to tell their story again.
Health professionals
Coordinate with the people in your care, maintain one shared record of treatment, and see the context you need before each conversation.
Registration asks for your licence details and a confirmation that you are a certified health professional.
What a shared plan holds
Everything below is written once and read by the people the care journey touches.
- Care goals
- What the journey is working towards, and the smaller steps behind it.
- Medication treatment plans
- What is taken, when, and what changed since the last review.
- Symptoms and side effects
- What has been felt and noticed, kept with the date it happened.
- Missed doses
- Gaps recorded as they happen, so a pattern becomes visible over time.
- Appointments and co-sessions
- Scheduled time, shared joining links, and who is taking part.
- Journals and progress
- Day-to-day notes, and how the whole picture has moved over months.
Joining for the first time
Registration takes a few minutes and asks only for what the person behind the account needs to know. Every new account is reviewed before access is granted, so you will get an email once yours is open.
Not sure which account you need?
